Boys and Girls Club Forms
All required forms for Boys & Girls Club programs can be found below. These include registration, permission slips, and special event forms.
Please review each form carefully before submitting. Completed forms may be returned to the Club office or emailed to pwalker@iowas.org.
All forms are good for the entire year, from the first day of school until the last day of summer programming. They need to be resubmitted each year.
Annual Club Membership
Used to enroll children in after-school and summer programs and includes emergency contact info. Children have a 3 day grace period to participate in our programming before the form is required.
Field Trip Permission Slip
Required for all off-site activities, including cultural outings and sports events. It includes transportation consent.
Family Handbook
For parents and guardians, this book explains our rules, policies, and procedures. There is a form on the last page to acknowledge your understanding.
Photo & Media Release Form
Grants or declines permission to use your child’s image in club media. Applies to photos and videos for newsletters, websites, and social media.
Health & Allergy Disclosure Form
Helps staff provide a safe environment by identifying medical conditions or allergies. Especially important for snack programs, physical activity, and field trips.
